Procedures
- The applicant shall submit the report of the security center, the official document, and a recent personal photograph to the Reception Desk.
- The enclosed documents will be verified for completeness.
- If the submitted documents are not complete, the applicant will be asked to update the required documents.
- If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
- The auditor endorses the voucher.
- The applicant then pays the prescribed fees at the cashier’s counter.
- The Applications Intake officer enters data.
- The auditor receives the application and validates information against it.
- In case of errors, the application is sent back to the Applications Intake officer to double check the entered dat
- In case the application is complete, the Audit and Posting officer will post the application.
- The application is sent electronically to printing department.
- The new card is printed and handed over to the Reception Desk officer.
- The Reception Desk officer then gives the new health card to the applicant.
Required Documents
- A report from a security center stating that the health card was lost.
- An official document from the original applicant (certificate of employment, a letter from the Ministry of Social Development).
- One (1) personal photograph.
